Tucker Home Healthcare LLC (“Tucker,” “we,” “us”) respects the privacy of patients, families, and referral partners in Greater Columbus. This policy explains what this website does with information, what it does not collect, and how to reach us.

This page is the privacy policy for the website. It is not the Notice of Privacy Practices that applies to medical records once someone is receiving care. Questions about a patient record belong on our HIPAA page or with the office.

Who we are

Information you choose to send

You can reach us without creating an account. This website has no patient portal, no login, and no payment page.

Forms. The consultation form, the contact form, and the Care Navigator ask for details such as your name, phone number, email address, the kind of support you need, and a short message. Those forms do not save your answers in a database on this website. When you submit one, your browser opens your own email program with a message addressed to Tuckerhhc@gmail.com. The message is sent only if you send it from that program.
Email, phone, and fax. If you email, call, or fax the office, we receive what you choose to send so we can respond and, if you ask, arrange care.

A first message only needs a name, a phone number, and a general description of the support you are looking for. Please do not put Social Security numbers, insurance ID numbers, or a full medical history in a website email. Call (614) 796-0939 when the conversation needs to stay on the clinical line.

Information collected automatically

We do not run advertising trackers or analytics tools on this site. The company that hosts the website may keep ordinary server logs, such as an IP address, browser type, and the pages requested, so the site can stay online and be protected from abuse. We do not use those logs to build advertising profiles.

The contact page shows an embedded Google Map of our office. When that map loads, Google may receive technical information such as your IP address under Google’s privacy policy. If you choose “Open in Maps” or “Get Driving Directions,” you leave this site and Google’s terms apply.

Cookies

This website does not set advertising cookies and does not use cookies to follow you across other websites. Your browser may store ordinary technical data that websites need in order to load. You can control cookies in your browser settings.

How we use information

We use information you send us to:

answer questions and schedule a consultation
understand which kind of in-home support a family is asking about
coordinate with the patient, the people the patient asks us to include, and the physicians involved in a plan of care
keep the website secure and working
meet record-keeping and legal duties that apply to a home health provider

We do not sell personal information. We do not share it for cross-context behavioral advertising.

Health information

If you include health details in a message, we treat that message as confidential and use it to respond to the request. Information gathered during care — visit notes, medication lists, physician updates — is kept in the patient record and is handled under the HIPAA Privacy Rule and our Notice of Privacy Practices, not under this website policy alone.

Who receives information

We share information only as needed for the reasons above:

your email provider, because the form opens a message in your own mail program
Google, because our office mailbox is a Gmail address and because the contact page embeds Google Maps
the company that hosts this website, for ordinary operation and security logs
clinicians, physicians, and family caregivers involved in a plan of care, once care is being arranged
a regulator, court, or other party when the law requires it

We do not sell, rent, or trade personal information.

How long we keep it

Emails and phone notes are kept long enough to respond and for ordinary business records. Medical records created after care begins are kept for the period required by Ohio and federal health-record rules. Server logs are kept for the period set by our hosting provider.

Security

We limit office access to messages and records to people who need them for care coordination. Email is a convenient way to start a conversation, and it is not a private medical-records system. Please use the phone for anything detailed or urgent. No method of sending information over the internet is perfectly secure.

Children

This website is meant for adults who are arranging care for themselves or for a family member. It is not directed to children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child under 13 sent us information through the site, contact us and we will delete it.

Your choices

You can decide not to use the forms and can call or visit the office instead. You can ask what we still have from a message you sent through the website, ask us to correct it, or ask us to delete it when we are not required to keep it. Some states, including California, give residents rights to know about, delete, and opt out of the sale of personal information. This website does not sell personal information. The site is written for families in Central Ohio. If you visit from outside the United States, your information is handled in the United States under this policy.

Changes

If we change how this website handles information, we will update this page and the effective date at the top. Continued use of the site after that date means the updated policy applies.
