Patient Rights
Patient Rights
Effective October 30, 2026. A plain-language summary for families in Greater Columbus.
If you receive care from Tucker Home Healthcare LLC, you keep the rights below. They follow the patient-rights standards used for home health in Ohio and under federal home health rules. Before care begins, your clinician gives you the full written notice and explains it in words you understand. This page does not replace that notice.
A family member or other representative may exercise these rights when you ask them to, or when the law says they may decide for you.
Dignity, privacy, and your home
- Be treated with dignity, courtesy, and respect. Clinicians are guests in your home.
- Have your personal property treated with respect.
- Be free from abuse, neglect, mistreatment, and misappropriation of property.
- Receive care without discrimination. See our non-discrimination notice.
Taking part in your care
- Take part in planning your care, including goals, the services to be provided, and who is welcome during a visit.
- Be told, before care starts and when the plan changes, what services you will receive, who will provide them, and how often visits are expected.
- Be told the name and role of the clinician in your home.
- Receive care from people who are licensed or otherwise qualified for the service they provide.
- Help choose visit times when scheduling allows.
Consent and refusing care
- Accept or refuse a treatment, a visit, or services, after the likely results of that choice are explained.
- End services. We will tell you what that decision may mean for your health and for any transfer or discharge.
- Be informed of Tucker’s transfer and discharge policies before those steps happen, except when a change is needed for your immediate safety.
Your health information
Your clinical record is confidential. We share it with you, the people you ask us to include, the physicians involved in the plan of care, and others only as the law allows. You may ask to see information in your record and ask us to correct it. Details are on the HIPAA page and in the notice you receive at the start of care.
You have the right to make advance directives, such as a living will or a health care power of attorney, and to have them followed as Ohio law allows. Tell us if you have one so a copy can stay with the record.
Cost
Before care starts, you may ask what we know about charges and about whether Medicare, Medicaid, or another plan is expected to pay. A conversation on this website is not a coverage decision. If your plan does not pay, we will explain what, if anything, you may be billed.
Raising a concern
You may voice a concern about a visit, a clinician, your property, or a service that was not provided. Raising a concern will not be held against you, and it will not by itself reduce the care you are entitled to receive.
Call (614) 796-0939 and ask for a care coordinator, or email Tuckerhhc@gmail.com. Our office is at 2700 E Dublin Granville Rd, Suite 280, Columbus, OH 43231. We will look into the concern and tell you the result.
You may also contact the Ohio Department of Health. You can remain anonymous.
- Complaint Unit: 1-800-342-0553
- Home Health Hotline: 1-800-669-3534
- Email: HCComplaints@odh.ohio.gov
- How to file: Ohio Department of Health, home health complaints
If your care is paid by Medicare, you can also call 1-800-MEDICARE (1-800-633-4227). A concern about a nurse’s license can go to the Ohio Board of Nursing. A concern about a physician’s license can go to the State Medical Board of Ohio.
What we ask of you
- Give us information that is accurate, and tell us when your condition, medicines, or wishes change.
- Tell us about advance directives and about people you want included in updates.
- Treat clinicians with the same respect you expect in your home, and tell us about hazards we should know before a visit.
- Follow the plan you agreed to, or tell us when you cannot.
