Effective October 30, 2026. These terms cover use of the Tucker Home Healthcare website.

This website is published by Tucker Home Healthcare LLC (“Tucker,” “we,” “us”) at 2700 E Dublin Granville Rd, Suite 280, Columbus, OH 43231. By using the site, you agree to these terms. If you do not agree, please leave the site and call (614) 796-0939.

These terms are about the website. They do not replace a plan of care, a physician order, or the rights you have once you are a patient. Those rights are described on our Patient Rights page.

What this website is

The site helps families in Greater Columbus learn about in-home nursing, therapy, and chronic-care support, and reach our office. It is general information. It is not medical advice, a diagnosis, a physician order, or a promise that a particular clinician, visit time, or insurance payment will be available.

If someone may be in danger, call 911. This website is not an emergency service. The office line is (614) 796-0939, Monday – Friday: 8:00 AM – 6:00 PM. Emergency Nurse On-Call 24/7.

Care is not started by the website

Reading a page, finishing the Care Navigator, or sending a message does not admit anyone to care and does not create a provider–patient relationship. Care begins only after our intake team speaks with you and the clinical orders that service requires are in place.

We serve families in Franklin, Delaware, Licking, Fairfield, and Pickaway Counties, and in nearby Central Ohio communities. We may decline or delay a request when we do not offer the service, the home is outside the area we can staff, orders are not in place, or we cannot arrange a safe visit.

How the forms work

The consultation form, the contact form, and the Care Navigator do not save what you type in a database on this website. Submitting opens your own email program with a message addressed to Tuckerhhc@gmail.com. The message is sent only if you send it from that program. If the email program does not open, call (614) 796-0939.

A first message should include a name, a phone number, and a general description of the support you need. Please do not send Social Security numbers, insurance ID numbers, or a full medical record through the form. How we handle information is explained in the Privacy Policy.

Insurance and payment

Nothing on this site is a coverage decision. Medicare, Medicaid, and private plans each have their own rules. A coordinator can explain what we know about eligibility after speaking with you and, when needed, with the physician. You are responsible for charges that your plan does not pay, to the extent the law allows us to bill you.

Using the site

You agree not to:

use the site for an emergency or to send someone else’s information without authority to do so
try to break, scrape, or overload the site
post or send anything unlawful, misleading, or harmful
copy the design, logo, or photographs for another business

The Tucker name, logo, photographs, and page copy belong to Tucker Home Healthcare LLC or its licensors. You may share a link to a page.

Other companies

The contact page embeds a Google Map, and some links leave this site for maps or email. Those services have their own terms. We are not responsible for a third-party site. Your email provider delivers form messages. Our office mailbox is a Gmail address.

The site can change

We may update pages, hours, or services, and the site may be unavailable at times. We will post changes to these terms on this page with a new effective date. Continuing to use the site after that date means you accept the update.

Limits

The website is provided as it is. We do not warrant that every page is complete or always available. To the extent Ohio law allows, Tucker Home Healthcare LLC is not liable for damages that come only from using the website, such as a message that did not send.

These terms do not limit liability that Ohio or federal law does not allow us to limit. They do not waive patient rights, professional duties, or duties under HIPAA once someone is receiving care.

Ohio law

These terms are governed by the laws of the State of Ohio, without regard to conflict-of-law rules. Disputes about the website are brought in the state or federal courts located in Franklin County, Ohio, except where a law gives you the right to a different forum.
